Scholarship Application

Pulaski White Rural Telephone Cooperative (dba LightStreamTM) is a non-profit corporation that was established in 1954 to provide telephone service to portions of Pulaski and White Counties. LightStreamTM is now the premier broadband provider in the area with a fiber optic network that offers gigabit internet.

LightStreamTM will award up to five (5) $1,000 scholarships to 1 or more students within the following school districts:

• North White
• Pioneer
• Twin Lakes
• West Central
• Eastern Pulaski

Applicants must meet the following requirements:

• Be a state-accredited high school or home schooled graduating senior
• Applicant’s parent(s) or legal guardian must be a customer as of the beginning of current school year and are a current customer of LightStreamTM
• Become a student attending an accredited college or technical school

• Complete the online scholarship application.
• Attach copies of high school transcripts or equivalent.
• Include a 1-page essay that describes your education goals, the importance of this scholarship, and what your next 10 years may look like.
• Attach a senior picture. Scholarship winners will be published.
• Deadline to submit is March 31.

• Winners will be selected by the Board of Directors
• Scholarship presentation may be at the applicant’s school awards program
• The LightStreamTM Board of Directors reserves the right to withhold any or all scholarships

• Awarding of payment will be submitted to the applicant’s accredited college or technical school after proof of 1st semester completion and proof of 2nd semester enrollment
• The award must be claimed by December 31 of the award year. A W-9 form will be submitted to the applicant in the year it is paid.
